This paper presents the process and conclusions of the Failure Mode and Effects Analysis (FMEA)that assesses the critical control points and risks of ultraviolet (UV) disinfection. The FMEA was completed tosupport the development of the interactive UV Disinfection Implementation Tool (UVDIT). The UVDITis being developed as part of the American Water Works Association Research Foundation (AwwaRF)Project #2861, Integrating UV Disinfection into Existing Water Treatment Plants. The FMEA resultswill be used in conjunction with the information gathered from a utility survey and power quality analysisto develop the UVDIT. The UVDIT will assist utilities in determining the feasible retrofit locations ofUV disinfection, the risks at the retrofit locations, method for mitigating those risks, and conceptualretrofit and risk mitigation costs.
